GAUGE: First row + 11 rows = 5 inches (12.7 cm) in Pattern.
BE SURE TO CHECK YOUR GAUGE. When you match the gauge in a pattern, your project will be the size specified in the pattern and the materials specified in the pattern will be sufficient. If it takes you fewer stitches and rows to make a 4 in. [10 cm] square, try using a smaller size hook or needles; if more stitches and rows, try a larger size hook or needles.

I am an expert crocheter - I have crocheted for many years and extremely complicated patterns of all kinds.
This pattern is so poorly written I don't see how anyone can crochet it from the instructions given. The photo doesn't show close enough detail to make it clear.
How can such a simple pattern be written so badly that I can't even understand it?
Bottom line: NOT WORTH THE TROUBLE IT TAKES TO READ THIS PATTERN!
